As a Ballad Health designated Community Health Improvement Site, FSWVA joined the Unite Us data platform to support community-wide care coordination between health and social service providers.
Through Unite Us, FSWVA can proactively connect people to our programs and services including the Harvest Home Community Kitchen, Mobile Marketplace, Mobile Food Pantry, and SNAP benefits to address food insecurity before it happens. We are now able to refer clients to the partner food agencies and other services.
The Food Assistance Referral Coordinator receives referrals and connects clients to services in their community.
Unite Us allows the Coordinator to track and monitor referrals from beginning to end to provide a more person-centered approach to service delivery.
